To request reimbursement, you must submit an itemized estimate/invoice. Please reach out to your Contractor for the detailed diagnosis and costs so that you may return to submit for reimbursement.
We will reimburse you in accordance with your authorization, subject to all dollar limits, exclusions, non-covered costs, or other coverage specifics in your Agreement. Any approved amount will be reduced by your Service Fee. Please allow 30–45 days to receive your reimbursement check.
